Centralized Admissions Coordinator
Were looking for a motivated, detail-oriented, and compassionate individual to join our Centralized Admissions Team, supporting hospital partnerships and helping patients access the care they need.

Position: Centralized Admissions Coordinator
Location: Primary location in Avon, CT. Must be willing to travel as needed.
Type:
- Full-Time Monday-Friday
- Rotating holidays and weekends
- Per Diem weekends
What Youll Do:
- Review referrals from hospitals for all our skilled nursing facilities
- Collaborate with hospital case managers and internal teams to ensure smooth, timely admissions
- Make clinical and financial admission decisions that help patients transition to the right level of care
- Plan, organize, and execute the admission process, ensuring compliance with all regulations and guidelines.
- Be part of a high-performing, fast-paced team that values accuracy, critical thinking, and a patient-first approach
What Were Looking For:
- Strong communication and problem-solving skills
- Strong attention to detail, follow-through skills, and ability to prioritize tasks.
- Ability to assess clinical information and make sound admission decisions
- Knowledgeable of admissions and discharge procedures, current laws, residents rights, and documentation requirements
- Experience in healthcare, case management, admissions, or related field preferred
- Someone who thrives in a team environment and is ready to grow with us
- Ability to remain calm in stressful situations and communicate professionally
