Community Care Connection Part-Time Family Nurse Practitioner (FNP) Or Physician Assistant (PA)
Pay: $52.00 - $58.67 / hour, depending on experience and qualifications
The Community Care Connection (CCC) Part-Time Family Nurse Practitioner (FNP) or Physician Assistant (PA) is responsible for providing high quality, patient-centered medical care and health education through the Community Care Connection mobile clinic (Care Coach) as part of the Rosalind Franklin University Health Clinic (RFUHC). This role supports the delivery of accessible healthcare services to underserved and vulnerable populations within the community.
The Family Nurse Practitioner or Physician Assistant will serve a vital role in advancing access to healthcare for underserved and vulnerable populations through the Community Care Connection mobile health program while also functioning as an integral member of the RFUHC Primary Care team. This position provides comprehensive primary and preventive healthcare services across the lifespan, including health assessments, diagnosis and treatment of acute and chronic conditions, health screenings, patient education, and care coordination.
Working collaboratively within an interdisciplinary healthcare team, the APP will help support the delivery of a Patient-Centered Medical Home (PCMH) model of care, promoting quality outcomes, continuity of care, disease prevention, and population health management. The successful candidate will demonstrate a commitment to health equity, community engagement, and evidence-based practice while contributing to the educational mission of Rosalind Franklin University.

Essential Duties & Responsibilities
- Travel throughout Lake County to community-based service locations with the Community Care Connection (CCC) Mobile Screening and Education Unit, providing clinical services approximately three to five days per week as scheduled.
- Maintain flexibility to support community healthcare initiatives, including occasional evening clinics (5:00 p.m. 8:00 p.m.) and up to two weekend outreach or screening events per month.
- Provide comprehensive primary and preventive healthcare services, including health assessments, wellness screenings, patient education, and follow-up care in accordance with established clinical standards, evidence-based practices, and organizational policies.
- Conduct patient evaluations, obtain and document comprehensive medical histories, perform physical assessments, and develop appropriate treatment and care plans within the scope of practice.
- Accurately document patient encounters, diagnoses, treatment plans, and evaluation and management (E&M) coding within the Athena Electronic Health Record (EHR) system.
- Monitor, assess, and document patient conditions, screening results, and outcomes, communicating findings and recommendations to patients and other healthcare team members as appropriate.
- Provide patient and family education regarding disease prevention, health promotion, medications, nutrition, wellness strategies, and recommended follow-up care.
- Perform and oversee community-based health screenings, testing, immunization initiatives, and other preventive health services delivered through the mobile unit and RFU Health Clinics.
- Participate in public health initiatives, including vaccination clinics, infectious disease screening programs, health education campaigns, and emergency response activities as needed.
- Coordinate and provide follow-up care for patients identified through community screenings, including referrals to primary care, specialty care, behavioral health services, and community resources.
- Collaborate with physicians, advanced practice providers, nurses, students, and interdisciplinary healthcare professionals to ensure high quality, coordinated patient care.
- Maintain effective relationships with community organizations, resource centers, local health agencies, and community leaders to improve healthcare access and address health disparities.
- Utilize knowledge of community health and population health principles to serve uninsured, underinsured, and underserved populations and connect patients with appropriate healthcare and social service resources.
- Ensure compliance with infection prevention, safety, quality assurance, and regulatory requirements, including maintaining a clean, organized, and clinically appropriate environment for patient care.
- Participate in quality improvement, quality assurance, and performance improvement activities designed to enhance patient outcomes, service delivery, and program effectiveness.
- Support the implementation of the Patient-Centered Medical Home (PCMH) model through coordinated, patient-focused care and population health management initiatives.
- Participate in departmental meetings, training activities, and continuing education opportunities to maintain clinical competency and professional licensure requirements.
Perform other duties and responsibilities as assigned in support of the mission and goals of the Community Care Connection program and Rosalind Franklin University Health Clinics.
Conditions of Employment

Required Education & Experience
- Master's degree from an accredited Nurse Practitioner program or Physician Assistant program.
- Graduate of an accredited educational institution recognized by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or the Commission on Collegiate Nursing Education (CCNE)/Accreditation Commission for Education in Nursing (ACEN), as applicable.
- Current certification as a Family Nurse Practitioner (FNP) through ANCC or AANP, or certification as a Physician Assistant (PA-C) through the National Commission on Certification of Physician Assistants (NCCPA).
- Current Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification preferred.
