Nurse Practitioner or Physician Assistant - PRN

Role Overview This position involves conducting comprehensive health assessments for members in their homes or care facilities, focusing on health, medical history, medication adherence, and social factors. The provider reviews and observes patient conditions, collaborates with healthcare teams, administers tests, and documents findings following established protocols. Key Responsibilities Perform adult/geriatric assessments, including medical history, diagnoses, treatments, and health education. Coordinate with physicians, case managers, social workers, and caregivers for follow-up and referrals. Administer point-of-care and lab-based testing as needed. Provide services at members’ residences, ensuring quality care and documentation. Qualifications & Skills Master’s Degree or relevant experience with NP licensure and prescriptive authority. Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care. Current BLS, ACLS, or CPR certification. Valid driver’s license, automobile insurance, and computer proficiency. Strong communication skills; new graduates encouraged to apply.
