Cadence is seeking a bilingual Spanish-speaking Clinical Navigator to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time, remote role is based in the United States with standard hours Monday–Friday. You will conduct outbound outreach, coach patients on behavior change, manage care plans, coordinate with clinicians, document interactions, and use Cadence Platform, Google Workspace, and Slack. #J-18808-Ljbffr Cadence
