Hospice Social Worker (Masters Level)

The Medical Social Worker provides emotional and psychosocial support services and linkage to community resources for individuals and families, based on an ongoing social worker assessment of need. The social worker plans, implements, and evaluates care through systematic assessment of psychosocial needs. Provides care and assistance to patients and families during on-call hours as scheduled. Shift 8:00 am - 4:30 pm, Monday through Friday, with other hours as needed for groups and events. The social worker plans, implements, and evaluates care through systematic assessment of psychosocial needs. Provides care and assistance to patients and families during on-call hours as scheduled. Shift 8:00 am - 4:30 pm, Monday through Friday, with other hours as needed for groups and events. Requirements Masters degree in Social Work from a CSWE-accredited program. Experience in a health care setting, prefer experience with terminal illness, palliative care or home health, and bereaved families. Current knowledge of medical terminology. Must have Social Work license in KY and/or OH. Personal qualities of empathy, responsibility, dependability and intelligence. Must demonstrate effective communication skills with team members, clients, and family members. Must have the ability to work independent of direct on-site supervision. Demonstrates leadership and teaching ability. Must arrange travel and be able to access client homes independently. Must observe universal precautions. Must carry state minimum in automobile liability insurance, have a valid drivers license, reliable transportation, clear driving record, pass a drug screening and background check. #J-18808-Ljbffr A Hiring Company
