Requirements: RN License; BLS; 2+ years of Case Management experience; nursys must be on profile; DEGREE ACCREDITATION MUST BE ON PROFILE.
Responsibilities:

• Provide case management services in a hospital setting.
• Coordinate patient care and discharge planning.
• Collaborate with healthcare teams to ensure optimal patient outcomes.
• Float to sister campuses as needed and operate as core staff.
• Adhere to facility protocols and documentation deadlines.
