What You’ll Do Provide high-quality, one-on-one physical therapy to geriatric patients in their homes. Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Implement effective plans of care to improve patient mobility and safety. Coordinate seamlessly with our nursing and clinical leadership team. What You Need Active Florida Physical Therapist (PT) License. Current CPR/BLS Certification. Valid Driver’s License and reliable transportation. Home Health experience preferred, but we will train the right clinician!
