- Conduct comprehensive occupational therapy evaluations for residents in a long-term care setting.
- Develop and implement individualized treatment plans to improve activities of daily living, mobility, cognition, and safety.
- Reassess progress regularly, adjust goals, and coordinate care with the interdisciplinary team.
- Provide patient and family education on exercises, compensatory strategies, safety, and use of adaptive equipment.
- Document evaluations, goals, progress notes, and discharge planning in the medical record.
- Ensure compliance with regulatory standards and participate in care planning and discharge planning. Requirements
- Ability to work in a long-term care/nursing home setting
- Comfortable with a day shift schedule (8:00 AM–4:30 PM)
- Strong documentation and communication skills
- Ability to collaborate effectively within an interdisciplinary team
