COMMUNITY HEALTH PARTNERSHIP OF ILLINOIS
A local non-profit health service organization is seeking a Community Health Worker to serve underserved populations. Responsibilities include linking individuals to health and social services, educating on health initiatives, and collaborating with community partners. Bilingual Spanish/English is required.
