Rotating on call after hours and weekends. Requirements: Master of Social Work (MSW) degree Minimum of one year of direct patient care experience in a community or home care setting Hospice experience preferred; EMR experience preferred Key Responsibilities: Participate in developing and updating each patient’s plan of care upon admission and when conditions change Ensure quality and safe delivery of hospice services Connect patients and families with appropriate community resources to address immediate needs Provide information about support services available to patients and families Assist families with mortuary arrangements as needed Conduct psychosocial assessments addressing emotional, social, and coping factors related to illness, prognosis acceptance, and the dying process Become part of our compassionate team, dedicated to providing exceptional care and support.
