We're seeking a detail-oriented RN with a passion for resident-centered care to join our team as an MDS / Care Plan Coordinator. As a key member of our interdisciplinary care team, you'll be responsible for ensuring our residents receive care that is both clinically sound and deeply personalized.

About the Role:

As our MDS / Care Plan Coordinator, you'll be responsible for:

• Coordinating timely and accurate MDS assessments

• Leading interdisciplinary Care Plan meetings

• Monitoring EMR documentation for accuracy and compliance

• Conducting in-services and training on care planning/documentation best practices

• Collaborating with physicians, therapy teams, and department leaders

• Ensuring compliance with state and federal regulations

What We're Looking For:

• Current Illinois RN License

• MDS experience

• AANAC certification (Preferred)

• Proficiency in EMR systems and documentation

• Excellent leadership, communication, and organizational skills

• A compassionate, resident-focused mindset
