Licensed Practical NursePH Ops of Arlington - Arlington, TX 76102Position SummaryThe LPN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse.Essential Duties and ResponsibilitiesUtilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomesInitiates and leads team conferences in development of individualized nursing care plansAssists with or institutes emergency measures for sudden adverse developments in customersContribute to the development of the customer/significant other plan of care based on the identified customer problems/needsCarry out physician orders accurately, directly and indirectlyAssist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needsDocument pertinent observation and intervention as appropriate in the EMRPerform selected tasks related to customer care and the customer's environment in a safe mannerAdminister medication accurately, observing customer response, as evidenced by documentation in the EMR and lack of negative outcomesPerform limited I.V. Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to LPN scope of practice.Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team feedback. Assist in assuring that effective communication exists with customer/significant other as reflected in the plan of careGive punctual change of shift report consisting of pertinent and relevant information for assigned customersEffectively communicate customer's changing needs and plan of care among team membersReport customer needs/problems and observations regarding customer's condition to Unit Manager, ARNP, and PhysicianReinforce customer/significant other information regarding plan of care and discharge plansWork collaboratively with other members of the health care team in coordination of the customer's care as evidenced by other department feedbackProvide customer/significant other health teaching as indicated by condition and needUse available written and AV materials specific to customer condition or diseaseUtilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussionDocument health teaching including customer/significant other response to and their understanding of health teachingParticipate in staff development programs as evidenced by education recordDemonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate resourcesComply with the infection control policies and procedures including ensuring a safe working environment for self and othersAttends and participates in in-service training.CompetenciesProvides therapeutic and rehabilitative care in accordance with nursing care planGives accurate and thorough report at change of shiftCollaborates with C.N.A. regarding patient's current and change of statusCollaborates with patient's Inter-Disciplinary Care TeamDelegates responsibility appropriatelyAccuracy of documentationTimeliness of documentation (real-time)Perform IV therapy on patients within LN scope of practiceAdministers medication accurately, as evidenced by the EMR and lack of negative patient outcomesEncourages patient growth and independence as care progression warrantsTakes a proactive approach to checking and stocking suppliesCultureThis is a high performance, ever-evolving environment propelled by collaboration through our Teams of Excellence (T.O.E.) The expectation for each team member is to engage in our complex and dynamic organization by adding value and support to fellow team members.Work EnvironmentThis job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility's no-smoking policy.Physical DemandsWhile performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds.Position Type and Expected Hours of WorkThis is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Must have an ability to learn new systemsAbility to work with a culturally diverse populationMust have positive communication skills and demonstrated adequate maturity and patienceProficient in the English language. Excellent communication (verbal and written), customer service and interpersonal skillsMust have good organizational skills with attention to detailMust be able to successfully prioritize workload, demonstrate initiative, and analyze situations to make sound decisionsMust be observant and recognize cleaning needsMust show initiative and take action on observed needsSelf-directing with the ability to work with minimal direct supervisionAbility to react decisively and swiftly in emergency situationsRequired Education and ExperienceHigh School Diploma or EquivalentGraduate of an accredited LPN programRequired License/CertificationValid State Appropriate LPN licenseCurrent CPR certificationPreferred Education and ExperienceA minimum of one (1) year of nursing experience in a SNF
