Medicaid Care Navigator — Field-Based Home Visits
Humana
Humana is seeking a Care Coordinator in Illinois to support the Medicaid HealthChoice program. You will assess members, develop personalized care plans, and connect families with resources to improve health outcomes. This field role involves 75–90% local travel and close collaboration with care teams. Reporting to the Manager, Care Management, you will work to ensure members receive appropriate services, navigate the system, and address gaps in care.
