Role Responsibilities

• Deliver comprehensive, ongoing healthcare for individuals and families across all age groups, from infants to seniors.
• Manage chronic conditions, provide preventive care, address acute illnesses, and respond to mental health needs with a patient-first philosophy.
• Perform physical examinations, analyze diagnostic tests, and create individualized care plans.

Education Requirements

• MD or DO degree from an accredited institution.
• Graduation from an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine.
• Eligible for (or currently hold) a valid Indiana medical license.
• Current BLS and ACLS certifications are desirable.
