RN Home Care – CHI Franciscan Health at Home • University Place, WA Job Summary and Responsibilities As a Home Health RN, you will: Be a clinical leader, applying your expertise directly in a patient’s most comfortable environment, delivering high‑quality, personalized nursing care. Craft individualized care plans, develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well‑being. Collaborate and coordinate with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care. Empower and educate patients and families, serving as a vital resource for teaching and emotional support. Mentor and guide – oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team. Job Requirements Graduate of an accredited nursing program with an active RN license in the state served. Minimum of 1 year of nursing experience (Home Health experience is a significant plus). Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient‑centered care. Current CPR/BLS certification. Valid driver’s license and reliable transportation.
