Gateway Medical , a division of Medical Services of America, Inc., is seeking an experienced ( Contract / Part-Time Nurse Practitioner) to join our team in the Greer, SC area.

• Potential for additional days/facilities based on Nurse Practitioner’s availability

• Compensation: $55 - $60 per hour

  Position Summary

  Typical day includes 8–15 scheduled visits within one assisted living facility, allowing for a manageable pace and strong patient relationships . The Nurse Practitioner provides high quality, compassionate care and support to patients and families through the application of advanced knowledge and skills. Working in collaboration with the Medical Director, this role focuses on services related to primary care, health promotion, and chronic disease management.

  • Provide primary care services within an assisted living setting
  • Manage chronic conditions and acute concerns
  • Perform Medicare Annual Wellness Visits and routine follow-ups
  • Collaborate with facility staff and care teams to optimize outcomes

  Responsibilities

  • Diagnose, treat, and manage acute and chronic illnesses.
  • Perform comprehensive health assessments and develop individualized treatment plans.
  • Routinely conduct preventative care visits such as Medicare Annual Wellness Visits.
  • Educate patients and families on health promotion, disease prevention, and care options.
  • Deliver consistent, competent care while ensuring a safe environment.
  • Assess patients’ living situations and advise on alternative options when medically necessary.
  • Provide counseling and support, for patients and families, regarding end-of-life care.
  • Order, conduct, supervise, and interpret laboratory and diagnostic imaging tests.
  • Prescribe pharmacological agents and non-pharmacologic therapies.
  • Maintain accurate, timely, and confidential records, using our EMR system.
  • Document all situations, both internally and externally, through our EMR to maintain accurate patient charts.
  • Participate in continued learning and annual continuing education as required by state licensure.
  • Collaborate as a key team leader and member of the interdisciplinary health care team.
  • Serve as a mentor, provider, researcher, manager, and consultant for best practices.
  • Ensure that patient rights and cultural treatment considerations are honored, as able.
  • Comply with legal, regulatory, and accreditation requirements.
  • Follow organizational policies and procedures for billing, documentation, and payroll.
  • Adhere to infection control protocols and use of PPE optimization, including in times of shortage.
  • Stay current with federal, state, Joint Commission, and organization regulations concerning patient care.

  Qualifications and Skills

  • Licensed Nurse Practitioner in South Carolina
  • Board certified in Adult-Gerontology Primary Care, Adult-Gerontology Acute Care, or as a Family Nurse Practitioner
  • Previous primary care experience preferred
  • Coverage in Greenville, SC area is anticipated approximately 1-2 times per year and may involve 1 additional work day during a single week.
  • Ability to commute to Greer, SC one day a week
