With over 25 years of service, we're a two-time Home Health Care Elite Award winner.Our clinicians choose San Diego Home Health because of our supportive culture, flexible scheduling, tenured leadership, and genuine sense of family.What You'll DoConduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical ManagerDevelop, implement, and update individualized plans of care based on patient needs and physician ordersPerform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home settingComplete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as neededComplete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicableObserve, document, and report patient response, progress, and any significant changes to the physician/provider and care teamCommunicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality careProvide education and support to patients and their families to promote safety, independence, and long-term successAssist with orientation, supervision, and consultation for Physical Therapy Assistants, as assignedYou'll Love This Role If You Are...Compassionate, dependable, and patient-focusedComfortable working independently in the home settingA collaborative team player who values communication and supportMotivated by meaningful, one-on-one patient careQualificationsMaster's degree in Physical TherapyActive Physical Therapy license in good standing in the state of CaliforniaAt least one year of experience working as a Physical TherapistMust have a vehicle, a valid current driver's license, and automobile insuranceAbility to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely mannerHome Health and Home Care Home Base documentation experienceSchedule OptionsWe are currently hiring for Per Diem Physical Therapy opportunities.
