As a Certified Nurse Aide youll contribute to our success in the following ways:

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing dressing grooming toileting and other daily activities.
• Take and record vital signs including temperature pulse respirations and blood pressure.
• Help patients safely walk transfer and use mobility equipment.
• Follow each patients individualized care plan.
• Monitor and report changes in a patients condition to the care team.
• Help maintain a clean safe and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation protect patient privacy and participate in required training.

What is Required

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
