Physical Therapist - Home Health - PRN - Northwest Columbus, OH

The role requires a licensed professional who provides coordinated care to patients of all ages, plans, implements, and evaluates patient care plans to restore or maintain well‑being, and collaborates with an interdisciplinary team. Position Specific Responsibilities Provide care using infection control measures protecting staff and patients (OSHA). Demonstrate commitment to professional growth and competence by attending required in-services and participating in in‑service programs. Accept clinical assignments that match education and competence for patient care. Monitor assigned cases to ensure compliance with third‑party payor requirements. Communicate effectively with patients/families, team members, physicians, and other health professionals to coordinate services and provide appropriate referrals. Perform initial and ongoing clinical assessments to identify patient needs, determine functioning level, conduct OASIS assessments at appropriate time points, and assess patient suitability for home health. Oversee and, when applicable, perform tasks with other team members to meet patient needs and preferences. Prepare and complete a Plan of Care in consultation with the physician and care team; document progress notes, phone orders, and other clinical records continuously, revising the Plan of Care based on assessment data. Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment. Assist or instruct patients/families in transfer techniques appropriate to patient abilities; instruct on use of walkers, wheelchairs, canes, etc., safely; utilize appropriate therapy equipment for the diagnosis and needs. Perform other duties as assigned. Education and Experience Bachelor of Science in Physical Therapy required. Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred. Skills Mathematical Skills: Ability to perform basic arithmetic, compute rates, ratios, and percentages. Language Skills: Ability to read, analyze, and interpret professional journals, technical procedures, and regulations; write reports, correspondence, and procedure manuals; present information effectively and respond to questions; strong written and verbal communication. Other Skills: Ability to understand, read, write, and speak English; articulate and embrace hospice philosophy. Certifications, Licenses, and Registrations Active and unencumbered Physical Therapist license in the state(s) of employment required. Current CPR certification required. Physical Demands and Work Environment The role requires regular standing, walking, and manual dexterity, with the ability to lift and move objects up to 50 pounds. Visual acuity to close, distance, color, peripheral, depth, and focus is required. The work environment may involve exposure to bodily fluids, infectious diseases, and other typical healthcare hazards. Routine use of standard medical equipment and tools associated with clinical care is essential.
