Physical TherapistWork Schedule:You may be eligible for a $20,000 sign on bonus.100% FTE, full-time. Day shift. RequiredLicenses & CertificationsCurrent license from the State of Illinois. RequiredBasic Life Support/CPR Certification. RequiredValid State driver's license and proof of liability insurance. RequiredAn applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
