Record daily billing and follow department guidelines for productivity. Prepare evaluations in accordance with department policies, procedures, and standards of practice, obtaining patient history, demonstrating knowledge of diagnoses, and researching unfamiliar diagnoses. Select and administer tests and measures. Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care. Establish treatment plan based on evaluation results, goals, and best practice; collaborate with interdisciplinary team, patients, and patient support system; assess and re‑assess need for modifications to plan of care and goals. Identify need for consultations and/or refer patient to another health care professional or service; initiate and complete discharge plan. Provide education and training to patient and/or support system, including ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription and training, and community reintegration. Responsibility for treatment, plan of care, exercise prescription for patient caseload. Provide interventions that are safe, effective, and in compliance with JAG standards. Musculoskeletal interventions, including therapeutic exercise, joint mobilization, and modalities. Write note data using Prompt EMR for all treatments provided. Oversee support staff of PTAs, ATCs, and PT aides. Attend annual JAG billing and coding seminar. Promote JAG in the community through lectures and presentations.
