Description

• Conduct assessments in both inpatient and outpatient settings and craft tailored treatment strategies for each patient
• Implement therapy modalities aimed at enhancing mobility, strength, balance, and overall functionality
• Engage with physicians, nurses, case managers, and multidisciplinary rehab professionals to achieve optimal health outcomes
• Guide patients and their families through therapy goals, aftercare strategies and home exercise recommendations
• Maintain thorough documentation of evaluations, treatments, and clinical progress as per institutional and regulatory standards
• Participate in holistic care team discussions and roundtable meetings to ensure seamless patient transitions

Education

• Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is encouraged
• Additional credentials in specialized areas (e.g., geriatric or neurological) are beneficial but not essential
