Clinical Pharmacist

POSITION DESCRIPTION: Provides ambulatory clinical pharmacy services, including disease management, comprehensive medication review, and Population Health Management under a collaborative practice agreement at multiple sites within the organization. Employment Type Full time Shift Day Shift Description Provides ambulatory clinical pharmacy services, including disease management, comprehensive medication review and Population Health Management at multiple sites. Essential Job Functions Provides pharmaceutical care by participating in patient care with the medical staff during outpatient clinic visits, designing, implementing and monitoring patient‑specific pharmacotherapy and promoting safe, evidence‑based, cost‑effective drug therapy. Performs Population Health initiatives, reaching out to patients based on pre‑determined clinical criteria to enroll them in pharmacist services, documenting interventions and outcomes for drug therapy quality improvement. Works collaboratively with clinicians, care managers, behavioral health managers, and other healthcare providers to implement disease‑state management programs, share best practices, and support quality improvement processes. Provides drug information and education to medical staff and patients, participates in drug use projects, cost reduction initiatives and quality improvement activities, and ensures efficient workflow adaptations. Maintains regular contact with staff at each practice location, attends provider and staff meetings, prepares for site audits, and serves as an educational resource and preceptor for pharmacy students or residents when needed. Essential Qualifications Education: Doctor of Pharmacy/Pharmacology degree required; completion of self‑management support training preferred. Credentials/Licensure: Valid, unrestricted state of Michigan pharmacist license; valid, unrestricted Michigan Controlled Substance License; valid CPR certification. Experience: Five (5) years of relevant experience preferred; PGY‑1 or PGY‑2 residency may substitute; Board Certified Ambulatory Care Pharmacist (BCACP) preferred. Position Requirements (Abilities & Skills) Knowledgeable in ambulatory patient care procedures, outpatient management of diabetes, hypertension, and hyperlipidemia, and medical terminology; able to perform dosage calculations and use desktop and Windows‑based systems including EMR, email, intranet, Microsoft Office, and other software. Strong written and verbal communication skills, phone etiquette, public speaking ability, collaboration in a team environment, professional image, confidentiality, compliance with privacy and best practices, organizational and time‑management skills, problem‑solving for reconciliation issues, and readiness to travel to multiple sites. Minimum Physical Expectations Regular keyboarding, phone work and charting; occasional walking, standing, bending, reaching, and lifting up to 5 pounds; adequate vision and manual dexterity for keyboarding, telephone, calculator, and office equipment; clear hearing and speech for telephone and face‑to‑face communication. Minimum Environmental Expectations Work in a medical facility with frequent walking throughout the hospital or clinic; exposure to patient care equipment, bloodborne pathogens, bodily fluids, sharps, and x‑rays; interaction with patients in distress or illness which may be stressful.
