Registered Nurse – Care Coordination / Case Management
Location: Sacramento, CA

Position Overview:
We are seeking an experienced Registered Nurse – Case Manager to coordinate patient care throughout the hospitalization while ensuring safe, timely, and effective discharge planning. The RN Case Manager will collaborate with physicians, nursing staff, social services, payers, and community agencies to facilitate appropriate utilization of healthcare resources, optimize patient outcomes, and ensure continuity of care. Candidates must possess strong acute care case management experience and be able to work independently in a fast-paced hospital environment.
Key Responsibilities:
- Coordinate comprehensive discharge planning for hospitalized acute care patients.
- Perform utilization review and evaluate the medical necessity of admissions and continued stays.
- Collaborate with physicians, nursing staff, social workers, rehabilitation services, and community agencies to coordinate patient care.
- Assess patients' psychosocial, financial, and discharge planning needs.
- Coordinate placement to Skilled Nursing Facilities (SNFs), rehabilitation centers, home health, hospice, and other post-acute care services.
- Work closely with Medicare, Medi-Cal, insurance providers, and managed care organizations to ensure appropriate authorization and coverage.
- Utilize Cerner , Midas , InterCall , Curaspan , and Microsoft Office for documentation, case management, utilization review, and discharge planning activities.

- 40 Guaranteed Hours per Week
- Every Other Weekend
- First to Float
- No On-Call Required
- 8-Week Travel Assignment
Requirements:
- Active California Registered Nurse (RN) License
- Minimum 2 years of RN Case Management/Care Coordination experience in an acute care hospital
- Minimum 3 years of hospital experience preferred
- Previous California Dignity Health/CommonSpirit acute care experience is required
- BLS Certification – Required
- Proficiency with Cerner, Midas, InterCall, Curaspan, and Microsoft Office
- Strong experience with discharge planning, utilization review, care coordination, and resource management
- Knowledge of Medicare, Medi-Cal, insurance authorization, SNF placement, and community resources
