Clinical Rehab Liaison

Encompass Health seeks a Clinical Rehab Liaison to support inpatient rehabilitation admissions and promote optimal patient outcomes. In this role, you will evaluate patients for rehab appropriateness, coordinate referrals and pre-admission screenings, and serve as the primary link between acute care hospitals, providers, patients, and families. You will educate stakeholders on rehab services, streamline transitions of care, and collaborate with an interdisciplinary team in a patient-centered, clinically advanced environment that supports professional growth and development.
