Requirements
- Active license as a Physical Therapist in the applicable state
- Basic Life Support (BLS) certification preferred
- Prior experience in a long-term care or skilled nursing facility preferred

Roles & Responsibilities
- Provide physical therapy services to maximize residents' functional abilities and independence in daily living activities.
- Evaluate residents and establish a therapeutic plan of care with functional goals.
- Consult with referring physicians and healthcare team members on care plans and discharge needs.
- Document all evaluative and therapeutic interactions in the Electronic Medical Record.
- Educate residents and caregivers on therapy goals and procedures.
- Collaborate with other therapy professionals to ensure resident care quality.

Compliance / Notes
- Maintenance of required licenses, certifications, and competencies is essential.
- Adhere to all infection control procedures.
- Participate in department orientation and staff mentorship.
