Remote Case Manager RN Telehealth Care Navigator
CVS Health
CVS Health is hiring a Remote Case Manager RN to support the Costco Team from home. You will assess member needs, develop care plans, coordinate resources, and collaborate with multidisciplinary teams to improve health outcomes.
This role requires an active multistate RN license, 3+ years of RN experience, and the ability to work CST hours Monday–Friday. Remote work offers flexibility and growth opportunities.
