Community Health Navigator — Access & Education

Southeastern Michigan Health Association (SEMHA)

Southeastern Michigan Health Association (SEMHA) in Wayne, MI, is seeking a compassionate Community Health Worker to join the Wayne FQHC Program. The CHW will help increase access to health services through outreach, education, and peer support within an inter-disciplinary clinical care team. The role emphasizes outreach, SDOH screening, referrals, and coordination with PCPs.
