Patient Navigator at Mass General Cancer Institute
Join a mission-driven team at the Mass General Cancer Institute (MGBCI) as a Patient Navigator! Essential functions include:
- Leading and participating in the intake process for new patients, helping them navigate their journey into MGBCI with compassion and clarity.
- Providing essential case management servicesincluding financial assessments, scheduling, and medical records coordinationto support patients and their families.
- Ensuring timely and accurate documentation in the electronic medical record, meeting expectations set by our Medical Director and Administrative Manager.
- Working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.
- Advocating for patients and their families, ensuring their needs and preferences are considered in the care planning process.
- Serving as a liaison between patients, healthcare providers, and other relevant stakeholders.
- Collaborating with the interdisciplinary healthcare team to coordinate patient care services, appointments, and follow-up plans.
- Assisting patients in understanding and adhering to their care plans.
- Identifying and connecting patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.
- Educating patients and their families about their medical conditions, treatment options, and self-care strategies.
- Ensuring that patients are informed and empowered to actively participate in their healthcare decisions.
- Maintaining accurate and detailed documentation of patient interactions, care plans, and resource referrals.
Qualifications:
- Bachelor's Degree required.
- Strong knowledge of healthcare resources, community services, and patient advocacy.
- Excellent communication and interpersonal skills.
- Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.
- Strong organizational and time management skills.
- Familiarity with electronic health records and case management software.
