Monitor vital signs, assess changes, and report variances to the supervising registered nurse. Assist residents with activities of daily living and promote independence and comfort. Implement and adjust care plans, perform basic wound care, and follow infection control and safety protocols. Maintain accurate charting and communication with the health care team to coordinate care. Ensure resident safety, fall prevention, and adherence to facility policies and regulatory requirements.
