Outpatient primary care delivered through telehealth Based out of the Pasadena clinic Care for patients across the lifespan Average daily patient volume of

patients Preventive care and chronic disease management Collaborative multidisciplinary healthcare team Modern technology and established clinical workflows Key Responsibilities

Conduct comprehensive patient assessments and telehealth evaluations Diagnose and treat acute and chronic medical conditions Develop individualized treatment plans and provide evidence-based care Prescribe medications and therapeutic interventions in accordance with California regulations Manage chronic diseases including diabetes, hypertension, asthma, and other common primary care conditions Provide preventive care, wellness counseling, and patient education Coordinate care with physicians, specialists, behavioral health providers, and support staff Maintain accurate and timely electronic medical documentation Participate in quality improvement initiatives and organizational goals Qualifications Required

Graduate of an accredited Family Nurse Practitioner program Current California Physician Assistant license Active, unrestricted DEA registration Master of Science in Nursing (MSN) Minimum of

one year of Physician Assistant experience

(clinical rotations do not satisfy this requirement) Fluent in English and Spanish , with the ability to independently conduct complete patient assessments without an interpreter Strong communication and patient relationship skills Preferred

Experience in community health, federally qualified health centers (FQHCs), or telehealth Passion for serving diverse and underserved patient populations Compensation & Benefits
