Social Worker - LCSW

Case WorkerUnder supervision, provides social services to clients. Identifies, develops, and implements social service plans for families and individuals. Determines eligibility for various services. May perform counseling and investigatory activities related to the work. Manages schedule to allow for adequate time for field travel, face-to-face patient engagement, completion of time-sensitive follow-up, documentation, and administrative tasks. Facilitates referrals to a variety of community-based services, including medical, mental health, substance use treatment, housing, transportation, home care, durable medical equipment, and respite care. Conducts post-discharge follow-up for patients when indicated. Provides backup coverage for facility placements and 24-hour discharge notice distribution.
