Job Description
Title: Physical Therapist - Rehabilitation

Schedule: Shift: 8 hours

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license and reliable transportation - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and determine appropriateness for therapy.
- Document and prepare/update care plans, progress notes, and clinical records on an ongoing basis.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
- Instruct patients and families on safe use of assistive devices and proper transfer techniques.
- Provide patient and family education regarding therapy procedures based on individual needs.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.

Compliance / Notes:
- Must meet mandatory continuing education requirements.
- Apply infection control and universal precautions during patient care activities.
