Case Manager - PT

Responsibilities Being assigned youth before their release from custody and remaining with them through the entire reentry process. Developing individual plans of care and delivering a wide array of support services to youth including educational, vocational and employment opportunities. Preparing records of client visits, progress, and case notes. Responding to emergencies and crisis interventions as needed. Establishing working relationships with families, court personnel, referral agencies and other service providers. Traveling in own vehicle and having mileage reimbursed. Working a flexible schedule including weekends, weekday evenings and daytime hours. Qualifications Bachelor's degree from an accredited college or university in human services, criminal justice or a related field. 1-3 years of case management experience (paid, volunteer, internship or practicum).
