Under the supervision of a pharmacist, the Clinical Pharmacy Technician will function in the critical role of obtaining an accurate medication history. This will include interviewing patients, utilizing software and contacting their outpatient pharmacies/facilities. The home medication list compiled by the Clinical Pharmacy Technician will have any discrepancies reconciled by a registered pharmacist to ensure patient safety and optimize pharmacotherapy.A day in the life of a Clinical Pharmacy Technician at Hackensack Meridian Health includes:Obtains accurate medication histories from patients in the Emergency Department or inpatient units as necessaryInterviews patients/patient's caregivers to obtain most precise data about the patients medications and supplements (e.g. name of medication, dose, frequency, route, last dose taken, last time refilled)Calls patient's outpatient pharmacy and or healthcare facility to obtain the best possible medication history (as above)Utilizes electronic health record to collect patient related information and document work done as necessaryCommunicates all medication discrepancies or complicated medication regimens to registered pharmacistUtilizes available resources to communicate with a translator if necessaryCommunicates with other healthcare professionals as necessaryDemonstrates an understanding of and adheres to all medication use and pharmacy policies and procedures.Performs other pharmacy technician duties as assigned.If Outpatient Pharmacy Technician is qualified, enter prescriptions into the order entry system.Other duties and/or projects as assigned.Adheres to HMH Organizational competencies and standards of behavior.Education, Knowledge, Skills and Abilities Required:High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.At least one year of pharmacy experience (outpatient or inpatient) required.Effective oral and written English communication.Strong ability to multitask.Ability and willingness to work cooperatively with others.Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.Licenses and Certifications Required:Pharmacy Technician License.PTCB Certification Required within 180 days of start of employment with the following exceptions:Hired prior to January 2024Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before 1-1-24.Licenses and Certifications Preferred:PTCB Medication History Certification - uncertified candidates are strongly encouraged to pursue certification after employmentCompensation Minimum rate of $25.77 Hourly HMH is committed to pay equity and transparency for our team members.
