Job Summary Has primary accountability for the delivery of high-quality clinical services to hospice patients. Assumes designated administrative oversight responsibilities in the absence of the Administrator and may report to the Chief Nursing Officer for administrative responsibilities. Essential Functions Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives. Demonstrates working knowledge of current regulations and standards including CMS, the Fiscal Intermediary, State of Michigan and other regulatory bodies and accrediting agencies. Appropriately delegates tasks to accomplish compliance and accreditation. Assists with the process for ensuring that Hospice services achieve and maintain health care accreditation and adheres to state and federal regulatory requirements, including face-to-face requirements. Performance is consistent with hospital commitment to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Is a liaison with field staff for general clinical problem resolution. Co-ordinates field staff and assists with joint visits with field staff as needed to verify competency, and problem solve. Oversees and coordinates the nurses, hospice aides and therapists to ensure efficiency and compliance with regulations. Perform skilled nursing visits in the field as needed. Provides backup for the Hospice Intake Process if needed. Assists with taking the information needed for new referrals, including any orders. Takes interim orders when needed. Serves as a liaison with physicians and the community when needed. Provides community education when a need is identified. Makes informational home visits, routine home visits, and admits patients to hospice services when needed. Provides high quality care including accurate and timely documentation. Coordinates the Interdisciplinary Team process and ensures that all patients are reviewed at least every 15 days as required by Medicare. Attends department meetings and other committee meetings as required. Participates in all required in-service and educational opportunities. Follows agency policies and procedures. Completes tasks in a timely manner. Participates in agency and system wide continuous quality improvement activities. Carries a caseload of four to six patients, when hospice census requires. Minimum Qualifications Bachelor of Science in Nursing degree preferred. Graduate of an accredited school of Nursing. Must have current, unencumbered license in the State of Michigan. Two (2) years’ experience in an acute care setting. (BLS from American Red Cross or American Heart Association) Knowledge, Skills & Abilities At least one year of hospice or other related experience preferred. Demonstrates good clinical skills and leadership abilities. Must have effective interpersonal skills in all areas of communication. Will spend up to 75% of work time in conversation with health care team members and customers. Must be able to work independently and be capable of solving problems with theoretical and practical approaches using good judgment. Must be able to work in a self-directed and collaborative environment. Has a working knowledge of federal and state regulations for hospice. Requires on-call responsibilities. Marshall, 200 N.
