Serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment. Pro-actively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity. Maintain a continuous dialogue with the care transition team to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options. What Minimum Requirements You’ll Need Licensure / Certification / Registration: Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
