About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 2 years of acute care case management or discharge planning experience required. Previous charge experience is preferred. Candidates must have hospital experience; behavioral health experience does not apply. License: California state license is required. Pending licenses are accepted but must be licensed upon start date. Certifications: Relevant nursing certifications are preferred. Must-Have: Experience with assessing patients, care coordination, and discharge planning. Ability to organize and participate in family meetings. Experience with health maintenance organizations, independent practice associations, etc. Knowledge of adoption/foster care, drug/alcohol abuse treatment options, and managing patients experiencing homelessness or mental health crises. Competent in arranging durable medical equipment, skilled nursing facilities, and ambulance transport independently. Description: The successful candidate will be responsible for case management and discharge planning within the acute care setting. They will coordinate care and ensure appropriate discharge planning. The candidate will participate in the development of treatment plans and advocate for patients' needs. This position requires participation in family meetings and collaboration with interdisciplinary support staff. Weekend shifts are required to meet departmental needs, specifically every other weekend. The candidate must be competent with computerized documentation. The daily patient census is approximately 25. Candidates must be flexible and willing to float between facilities as needed.
