As a Home Health Care Specialist, you will: Build and maintain strong referral relationships with healthcare providers and community partners. Generate new referrals and manage existing accounts. Develop relationships with RN case managers, discharge planners, physicians, and other healthcare professionals. Collaborate with clinical and operational teams to support patient care and ensure a smooth referral process. Attend sales meetings and interdisciplinary team meetings. Represent Lorian Health professionally within the local healthcare community. Required Qualifications Bachelor’s degree in a related field or five (5) years of previous sales or business development experience in home health or a related healthcare industry. Minimum of one (1) year of recent experience within the last three (3) years in a Home Health or Hospice setting. Working knowledge of Microsoft Office, including Word, Excel, and PowerPoint, as well as computerized systems in general. Strong knowledge of the local medical community preferred. Strong leadership, organizational, interpersonal, and communication skills, both written and verbal. Ability to communicate and present comfortably in front of small or large groups. Strong relationship-building and customer service skills. Pay Range: $90,000-$120,000 commensurate with experience.
