Behavioral Case Manager (MHP) Job Title: Behavioral Case Manager (MHP) Department: Mental Health Reports to: Wellness West Team Lead Job Type: Full Time, Salary, Non-Exempt, Grant Funded Job Location: Chicagoland area Salary Range: $38,000 – $49,000 About Rincon Rincon is a provider of high-quality and culturally competent services focusing on counseling and crisis intervention, substance use prevention and treatment, violence prevention, education, case management, community and family services. Position Overview The MHP works as part of a multidisciplinary team, delivering supervised therapeutic and rehabilitative services, case management, and integrated dual-diagnosis support. The role is community-based, meeting clients where they are and collaborating with Wellness West partners to ensure immediate needs are met. Responsibilities Assessment, Evaluations & Treatment Planning Conduct and document a holistic, trauma-informed integrated assessment, including mental status exam and supporting tools, resulting in a diagnosis confirmed by a licensed professional. Evaluate risk of harm and develop safety plans as needed. Assist clients with metric-based evaluations of symptoms. Collaborate with clients and support networks to develop individualized treatment plans and recommend services toward goals. Review Integrated Assessment and Treatment Plan (IATP) in compliance with regulations. Service Delivery Attend hospital discharges of Wellness West clients within required 90‑minute timeframe. Provide transportation to patients when necessary. Focus on case management and community support recovery needs. Follow treatment goals and collaborate with peer support specialists to meet immediate needs. Ensure health and safety needs are met. Deliver services in a dignified manner, complying with payer, licensing, and accreditation procedures. Coordinate care with internal and external providers as appropriate. Crisis Response Assist in de‑escalation of clients experiencing a mental health crisis. Establish rapport and strengthen engagement throughout crisis programs. Model positive recovery behaviors. Provide skill building and case management for clients with identified needs. Community Engagement Participate in outreach activities to offer services and promote professional development. Develop and maintain linkages with community-based organizations. Advocate for clients and the agency within broader health and social service systems. Document community engagement efforts in required formats. Provide services in the community as directed by grant requirements. Supervision and Oversight Participate in regular supervision from an RFS LPHA; minimum one hour of documented supervision per month. Ensure services adhere to IATP and all regulatory standards. Documentation and Compliance Document services within 48 hours of delivery. Maintain records meeting agency and regulatory standards. Participate in quality assurance and improvement activities. Adhere to HIPAA, 42 CFR Part 2, and corporate privacy policies; report any PHI breaches promptly. Professional Development Complete required agency and regulatory training—including state rules, sexual harassment, fraud, mandated reporting, and clinical courses. Maintain relevant licenses and certifications. Perform additional duties as assigned. Benefits Health, dental, and vision insurance. Life insurance. The role requires flexibility, independence, and coordination of transportation, including public transit or rideshare services. Schedule 8:30 am – 4:30 pm, Monday‑Friday, with on‑call availability 8 am – 8 pm for at least 16 days per month to respond to hospital discharges. Language Fluency in Spanish; bicultural applicants strongly preferred. Qualifications Strong case management and planning abilities. Excellent listening, communication, and client‑centered skills. Proficiency in de‑escalation techniques. Understanding of behavioral health needs in service areas, including co‑occurring disorders. Knowledge of community, medical, and supportive services. Collaborative mindset across differing contexts. Effective teamwork with peers, clients, and groups. Desire to learn new skills. Accurate and efficient record‑keeping. Position Requirements Bachelor’s degree in a social service field or five or more years of experience. IM+CANS certification preferred. Demonstrated ability to assess risk, gather histories, evaluate relationships, conduct integrated mental health assessments, use diagnostic tools, and under supervision write and implement treatment plans. Pass background checks, including DHS clearance. Full‑time use of a licensed, insured vehicle with a valid driver’s license, or proven travel independence.
