RN Care Manager

Humana is seeking an RN Care Manager in Eau Claire to coordinate holistic, member-centered care for diverse populations. In this role, you will complete clinical assessments, create and update individualized care plans, and close gaps in care by partnering with physicians, social workers, and community resources. You will educate members and caregivers, advocate for their needs, and address social determinants of health. Working in Humana’s inclusive, collaborative culture, you will use analytics and care-management tools to improve outcomes, support value-based care, and help members achieve their best health.
