PharmacistThis 0.0FTE/Per-Diem position is scheduled to work 10 HR variable shifts as needed and is UFCW represented.Compensation: Non-exempt, $70.71/HR-$115.25/HRMason Health also offers these benefits for per-diem employees:Mental Health & Wellness Resources – Access to Talkspace and the Calm App.Employee Assistance Program (EAP)Paid Sick LeaveJob SummaryThe pharmacist is responsible for ensuring appropriate pharmaceutical care for patients of Mason Health.Job Specific Duties & ResponsibilitiesAnalyze inpatient and outpatient medication orders for completeness and appropriateness as it relates to the 6 rights: person, medication, dose, route, schedule, and documentation.Supervise the work of the pharmacy technicians and provide support help as needed.Ensure appropriate distribution of medications for both inpatients and outpatients. This includes but is not limited to sterile compounding, bar code scanning, and automated dispensing devices.Analyze lab values to perform pharmacokinetic dosing, renal dosing, and therapeutic drug monitoring according to established protocols and current guidelines.Document therapeutic drug monitoring decisions and clinical interventions into the electronic medical record.Ensure appropriate medication histories are gathered for medication reconciliation.Dispense medications and maintain appropriate records according to state and federal regulations.Answer questions and provide information to clinicians and patients regarding drug interactions, side effects, dosages, and storage of pharmaceuticals.Respond to emergency response codes in the hospital and maintain ACLS certification.Provide assistance to other healthcare practitioners in coordinating nutrition support (including total parenteral nutrition).Assists with development of drug protocols and standing orders.Participates in Quality Assurance and Drug Use Evaluation programs and completes drug utilization reviews for the Pharmacy and Therapeutics Committee.Other duties as assigned.Required Education and ExperienceDoctorate in Pharmacy (Pharm.D.) preferred or 5 years' experience in a hospital or residency.Required Licenses, Certifications and/or RegistrationsCurrent license as a registered Pharmacist in the State of Washington.Mason Health practices equal employment opportunity towards all workforce members and applicants for employment.
