About the Role:
As an LPN/RN MDS Coordinator, you will play a vital role in ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments. Your responsibilities will include:
Coordinating the MDS process from start to finish, ensuring compliance with federal regulations and company protocols
Assessing resident needs and developing individualized care plans
Collaborating with interdisciplinary teams, including physicians, nurses, and other healthcare professionals
Maintaining accurate and detailed records, including MDS assessments, care plans, and progress notes
Providing exceptional customer service and support to residents, families, and team members
