- Conduct thorough evaluations of patients, order necessary diagnostic tests, and create personalized care plans
- Diagnose and manage acute, episodic, or chronic health issues in coordination with physicians or autonomously (as per credentials)
- Educate patients and their families on health promotion, preventive care, and management of diseases
- Prescribe medications and treatments adhering to scope of practice
- Engage in patient follow-ups and management of chronic conditions
- Work collaboratively with interdisciplinary teams to ensure comprehensive and holistic care
- Maintain precise documentation within electronic health records (EHR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Completion of an accredited Nurse Practitioner program
- Current Nurse Practitioner licensure in the state of Indiana
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (or ability to obtain within 90 days)
- Strong clinical skills complemented by a caring, empathetic demeanor
- Outstanding communication skills with patients, their families, and healthcare staff
- Flexibility in dynamic, patient-focused environments
- Familiarity with EHR systems and ease with telehealth technologies
