What You'll Do

As a Home Health Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN), you'll provide skilled nursing care to patients in their homes while working alongside an experienced interdisciplinary team. You'll play a vital role in helping patients recover, manage chronic illnesses, and maintain their independence.

Your Responsibilities

• Perform ongoing patient assessments and report changes in condition to the supervising RN and physician.
• Administer medications, injections, and prescribed treatments safely and accurately.
• Provide wound care, catheter care, and other skilled nursing procedures.
• Educate patients and caregivers on medications, disease management, safety, and home care techniques.
• Monitor patient progress and communicate updates with the care team.
• Document all patient visits accurately and timely using the electronic medical record (EMR).
• Ensure compliance with Medicare, agency policies, and infection control standards.
• Deliver compassionate, patient-centered care while maintaining patient dignity and confidentiality.

What We're Looking For

Minimum Qualifications

• Current, unrestricted LVN/LPN license in the state of practice
• High school diploma or equivalent
• Current BLS/CPR Certification
• Minimum 1 year of nursing experience (Home Health experience preferred)
• Valid driver's license, reliable transportation, and current auto insurance
• Ability to travel throughout the assigned service area

Preferred Qualifications

• Home Health, Hospice, or Community Health experience
• Knowledge of Medicare Home Health regulations a plus
• Experience using HomeCare HomeBase or other EMR systems
• Strong organizational and time-management skills
