Responsibilities

Responsible for delivering nursing care to patients in a long-term care facility. Responsible for monitoring patient conditions and responding appropriately. Communicate effectively with the nurse leadership and other members of the healthcare team regarding patient conditions. Appropriately assigns duties to Certified Nurse Assistants based on patient needs, staff availability and unit needs.

Qualifications

Certificate from an accredited Licensed Practical Nurse Program. NYS certified LPN license. Good clinical patient care and communication skills. Must be service-oriented with excellent interpersonal skills. Ability to read and write in English is required.
