Pride Health is hiring for an RN Care Coordinator for a client in Houston, TX! (Contract)
Zip: 77014
Schedule: Mon-Fri 8-5pm
Contract: 3 month contract
Pay Rate: $40-45/hour
Specific Duties:
- Setting: Family Medicine
- Coordinate patient care plans across providers, departments, and services.
- Perform clinical triage via phone, EMR messaging, or in person.
- Review labs, imaging, and clinical results and escalate appropriately.
- Prepare patients for procedures and ensure pre‐op requirements are met.
- Educate patients on treatment plans, medications, and follow‐up care.
- Manage referrals and authorizations (especially in outpatient settings).
- Document all clinical interactions in the EMR accurately and timely.
- Serve as primary point of contact between patient, family, and care team.
- Monitor patient progress and intervene when care gaps appear.
- Support quality‐improvement initiatives within the department.
Must have qualifications/experience:
- Bachelor's Degree in Nursing preferred
- RN License
- BLS Certification
- Experience in Family Medicine
- Strong patient‐education and care‐coordination skills
- EMR proficiency (Epic)
- Experience working in multidisciplinary teams
- Ability to manage high patient volumes and complex cases
- Care coordination or case management experience (1+ year preferred)
