Position Overview

As a Hospice RN Case Manager, you will:

• Coordinate and oversee the care of patients on your caseload
• Conduct nursing assessments and develop individualized care plans
• Provide direct patient care, education, and emotional support to families
• Collaborate closely with the interdisciplinary hospice team
• Ensure comfort-focused, compassionate end-of-life care
• Participate in on-call rotation as needed
