Job description: Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Noble Man Care Services Location Spring Hill, FL (Serving Hernando, Citrus, Levy, Sumter, Marion, and Lake counties) Employment Type: Full-time/Part-time Schedule: Mon-Fri 10 AM-6 PM, with 24/7 on-call rotation About Noble Man Care Services Join our innovative family-centered companion care and clinical home health hybrid agency. We provide premium clinical oversight with 24/7 APRN availability, serving clients with complex chronic conditions including diabetes, heart disease, hypertension, and post-operative care needs. Position Overview We're seeking a compassionate and skilled LPN or RN to join our growing team. You'll provide direct patient care in home settings while working under the supervision of our experienced APRN owner with 25+ years in healthcare. Key Responsibilities Clinical Care: Vital sign monitoring, blood sugar testing, medication administration, injections, wound care, dressing changes Specialized Care: Ostomy/catheter care, chronic condition management, post-surgical care Documentation: Complete OASIS assessments, care plans, and status reports using Alora, Charm Health, and eClinical systems Patient Education: Medication compliance, disease management, fall prevention Emergency Response: Participate in 30-minute urgent response protocol Quality Assurance: Participate in QAPI meetings and continuous improvement initiatives Required Qualifications Current Florida LPN or RN license CPR BLS certification Home Health Aide certification preferred Experience with chronic disease management (diabetes, hypertension, heart disease) Proficiency with wound care and post-operative care Reliable transportation and valid driver's license Preferred Experience Home health or community health experience Experience with OASIS assessments Familiarity with EHR systems (Alora, Charm Health, eClinical) Bilingual capabilities (Spanish preferred) What We Offer Competitive hourly rate ($25-35/hour based on experience and certifications) 1:1 caregiver-to-client ratio for personalized care Comprehensive malpractice insurance coverage Ongoing continuing education and training opportunities Supportive team environment with experienced APRN leadershipOpportunity to make a real difference in patients' lives Why Choose NMCS?
