Registered Nurse (RN) Long Term Care (PRN)

Registered Nurse (RN) – Long Term Care Schedule: 12-hour shifts as needed. Responsibilities Perform initial and ongoing assessments of patient condition. Establish and implement a plan of care appropriate to identified problems and patient preferences. Deliver plan of care interventions, including medication administration, specimen collection and clinical treatments. Document patient care performed. Monitor, record and communicate patient condition to the care team, physician, patient and family. Assist patients with activities of daily living such as personal hygiene, elimination, nutrition, and ambulation. Collaborate across disciplines to coordinate patient care, including transfers, discharge, referrals and spiritual/psychosocial support. Identify patient and family learning needs and provide education appropriate to age, culture, condition and circumstances. Advocate for the physical and emotional well‑being of patients. Qualifications Current Tennessee Registered Nurse license. Basic Life Support certification at the time of hire.
